Our TDDI / Complex Care service supports people to manage, improve or recover from a diagnosed health condition. It includes support with illnesses, long‑term conditions, disabilities or injuries. At its heart, TDDI is a safe, skilled, person‑centred approach that promotes wellbeing, prevents deterioration and helps people stay as independent and/orcomfortable as possible.

We provide personalised support for individuals who need treatment or ongoing management of a disease, disorder or injury. Our team which is led and managed by a qualified nurse, works closely with healthcare professionals and families to deliver safe, consistent care tailored to each person’s needs.
