Domiciliary Care

All services provided are dependent on an assessment.

Our Home Care service provides support to our clients which allows us to relieve the stress from family members or friends providing care while allowing our clients to remain in their own home for as long as possible.

 

Our visits can vary from a few hours a day to live-in carers. One or more visits can be made throughout the day from Monday to Sunday. All services provided are dependent on an assessment of needs and this is agreed with you and/or your family members.

Residential & Nursing Home Support

We aim to provide a service that is reliable and responsive to our clients, promoting independence, maintaining dignity and confidentiality.

​

We provide short and long term staff cover to supported living and residential homes which care and support vulnerable children and adults with learning disabilities including children and adults with learning disabilities and autism spectrum disorder (ASD) and/or attention deficit hyperactivity disorder (ADHD). We also support residential and care homes providing care to the elderly as well as homeless shelters and substance abuse.

The staff over can be for a few shifts a week or continuously on a daily basis over many months for one or multiple staff.

Live-In & Companionship Care

Live In Companion Care provides help and support as well as companionship, allowing older adults to remain independent and living in their own homes.

Our live-in care services ensure that a valued member of our team is with you 24 hours a day, providing the agreed support to enable you to live as independently as possible.

Supported Living for Adults with Learning / Physical Disabilities

Support and care is tailored to individual needs through a structured care and support plan.

 

Our flexible and varied range of services providing disability support includes: supported housing, supported housing with care, respite care, and nursing or residential homes. Support time can vary from a few hours a week, up to full-time support or intensive support over a short period of time to help achieve a goal.

We provide expert tailored support and care for people aged 18 or over with mild, moderate, or severe disabilities, including:

  • Autism

  • Sensory disabilities

  • Physical disabilities

  • Acquired brain injury

  • Associated mental health needs

  • Behavioural challenges

Personal Assisted Transport Service (PATS)

You and our doctor will determine your care plan and services you may need at home.

Homely Health Care is offering this new transport service for our clients, whether you are in your own home or in a nursing or residential care home.

Homely Health Care will provide you with one of our experienced carers (Personal Assisted Transport Service staff) who will pick you up from your address and accompany you to your hospital, GP, dentist, optician, hearing, or other appointments. Our PATS staff will stay with you from point of pick up to point of return.

 

All our PATS staff have experience in social care and aim to provide you or your residents with a personal service from the moment of pick up to the moment we return you or your residents.

 

All our PATS staff have:

  • Current enhanced DBS checks;

  • A full UK driver's licence;

  • Business care insurance;

  • Training including in Safeguarding vulnerable adults, person-centered care, and principles of care and confidentiality.
